ONLINE TOURNAMENT REGISTRATION FOR ADVANCING TEAMS
TEAM CHECK-IN/REGISTRATION CHECKLISTS
A FRIENDLY REMINDER: Please arrive with all your paperwork complete, and printed, properly collated/ordered (see checklist for required order and organization), with all the appropriate quantities of copies of each form as specified in the checklist. We want to ensure the most positive and expedient registration/check-in experience possible, and our check-in/registration sites will NOT be able to provide computer access, printer access, or copier access. You must arrive with all paperwork fully complete, accurate, and in order.
TOURNAMENT FORMS
All the forms outlined below are required for verification and/or submission at check-in/registration at Lone Star Finals. Appropriate formats and quantities will be verified at check-in/registration for the Tournament Data Forms and Expense Reports; however, many items will be officially submitted to appraisers at your team challenge on Saturday. The Declaration of Independence form requires two copies. One copy will be submitted at your team challenge site before your team competes/performs. The second copy will be presented at your check-in at the Instant Challenge site.
Out of an abundance of caution and preparation, Texas DI always suggests you bring a few additional copies of your required, completed paperwork.
FORMS REQUIRED FOR ALL TEAMS

Declaration of Independence (Minimum of 2 copies required for check-in/registration; 3 copies suggested. One copy delivered at team challenge performance site, and one copy at Instant Challenge site.) Not required for Rising Stars teams.

Expense Report (Minimum of 1 copy; 2 copies suggested. Verified at check-in/registration. Submitted to check-in appraiser at team challenge performance site.) Not required for Rising Stars teams or Texas Xtreme teams.

Parental Consent & Photo Release (minimum of 1 signed copy per team member ; 2 copies per team member suggested.)

Medical Release (minimum of 1 signed copy per team member; 2 copies per team member suggested.)

Team Sign (1 free-standing sign identifying your team is required, in accordance with the Rules of the Road. It is not required to be physically present at check-in/registration, but it will be verbally confirmed upon check-in/registration.)

Individual, Team Clarifications (If you received individual team clarifications throughout the tournament season, a minimum of 6 copies are required for Lone Star Finals; 9 copies are suggested. These are verified at check-in/registration, and submitted to the check-in appraiser at the team challenge site.)
